When you schedule your first appointment, we will send you a new patient packet that contains the paperwork needed for your first visit.  Please complete these forms and bring them with you to your appointment. This will help us better understand your medical history and the problems you are experiencing. You may also download your new patient packet here (PDF)

We ask that you arrive a minimum of 15 minutes before your scheduled appointment to allow us time to prepare your chart for the physician and make sure that we have complete and accurate insurance information.

You should expect your first appointment to last approximately one hour.  You will meet with one of our fellows who will obtain a thorough medical history and discuss the reason for the visit. We will likely need to obtain a urine specimen and a pelvic examination will be performed.  There may be other office procedures needed but those would be explained to you by the physician prior to performing them.

Your visit will usually end after a consultation with one of the attending physicians who will review with you again findings, treatment options and risks and benefits.  Because the consultation can be quite in depth depending on the findings we do ask that you bring someone that is supportive and that you are comfortable with to be in with you during the consultation.
